Ok I'll have a look and see where I got it from. Sorry about that. I'll be more cautious in the future.
EDIT: Turns out I got it from a download site. I found the cover on a few other download sites with reverse image search, but couldn't find any art/design sites. I may be wrong though. If you do get a complaint feel free to take it down, I appreciate your cooperation.

The back looks like it's custom from another cover site. You may want to be more cautious of where you get your covers. If you don't have permission to upload it from the original author I may have to take it down if someone complains. Other than that it's a great use of the uPlay template.
